What is the name, address and phone number of the veterinarian currently seeing your animal?
Are there a current veterinary health certificate and a complete inoculation record on the pet for sale?
Has your pet been checked, x-rayed and found free of a breed associated diseases by a licensed veterinarian?
Does your pet have a pedigree?
What is the pedigree of your pet?
Do you have registration documents?

If pure bred what is the ranking of the parents.
Is your pet registered with a national or international registry?
Does your pet conform to AKC/TICA/CFA etc. breeding standards?

What diseases and health problems are associated with this breed?
Is your pet on a special diet?
What is your pet's favorite food?
What do you currently feed your pet?
Do you give your pet any special supplements?

Is your pet suitable to be around children?

Is your pet intact, neutered or sprayed?
Has your pet been tattooed or micro chipped?
Has your pet ever been in a national or internationally sponsored show?
Has your pet ever placed in an AKC/CFA/TICA etc. sponsored event?
Have you ever been filed against by any kennel club, if so, what was the circumstance?
Is your pet being sold under a breeder or show contract with full registration, or as pet quality with limited registration?
Has this animal ever had any form of elective surgery, such as tail docking, ear cropping, dewclaw removal, declawing, etc.?
Has this animal ever been treated for any medical condition which required veterinary attention such as: poisoning, distemper, parvo, being hit by a car, lacerations, broken bones, anemia, fleas/ticks, lyme, kennel cough, weight loss, kidney/liver disease, heart problems, etc.?
Does the weight/height/conformation of the dog conform to the AKC standard of breed?
Is your pet an indoor cat?
Has your cat been tested for FelV/FIV?
Will a health certificate be part of the purchase agreement?
Are you willing to ship/deliver this animal to where I live? If so, what are the charges?
